Dear Parents/Guardians, St. Croix Regional Health Center will soon be receiving a limited supply of the Pfizer COVID-19 vaccine designated for local area students age 12 and above. SCRHC has asked the area schools to reach out to families to provide this information and determine if any parents/guardians are interested in having their child receive the vaccine. Students do not need to be current patients of St. Croix to be eligible. Should you be interested in having your child vaccinated, please complete the attached patient registration and consent forms. These forms must be returned to AES before Tuesday May 18th as vaccines are delivered on a first come first serve basis. Nurses will be available for administering vaccinations in schools on Friday May 21 for the first dose and Friday June 11 for the second dose. Please see the attached forms for more information on the Pfizer COVID-19 vaccine, or call the St. Croix Regional Health Center at 796-5503. Sincerely, Trevor Flood Principal |
